"Stat" multiplex polymerase chain reaction: its role in clinical microbiology.

چکیده
In the laboratory, the department most affected by the introduction of molecular diagnostic tools has been the clinical microbiology laboratory. For organisms such as Clostridium difficile, polymerase chain reaction (PCR) is the gold standard for diagnosis. Incorporation of molecular diagnostic techniques into microbiology laboratories of differing sizes has been hastened by the development of simplified technological methods, individual cassettes incorporating all reagents necessary for diagnosis, and instruments allowing multiple tests to be run at the same time. Companies, such as Cepheid (Sunnyvale, CA), with its GeneXpert technology , even advertise that their tests can be run in a physician's office. Pressure by the US Food and Drug Administration (FDA) on companies such as Roche has pushed them to change their offerings in molecular diagnosis from analyte-specific reagent kits to a commercially prepared, prepackaged assay with detailed instructions for performance of the test. This change has hastened adoption of molecular methods into the testing menu of laboratories that do not have the resources and staff to perform the extensive and time-consuming validations of assays that are not approved by the FDA. Molecular methods have probably found their greatest use in microbiology laboratories in the diagnosis of viral infections. The reasons for the movement to molecular methods in this arena are varied. The rapid assays for agents such as influenza A and B and respiratory syncytial virus have low sensitivity and specificity. Direct fluorescent antibody assays require trained technologists and are generally only available during the daytime and early evening hours. Culture is less sensitive than molecular assays. The main advantage of molecular assays over conventional methods is that they can detect newer viruses that are often unculturable. Multiplex assays that detect multiple organisms in a single procedure are joining singleplex tests that detect nucleic acids from a single species. However, even the newer multiplex PCR tests, which have high sensitivity and specificity, are complicated, are technically demanding, and require highly specialized technologists. The turnaround time for these assays is fairly long (ie, 6-8 hours). As a result, these tests are generally only performed during daytime hours in virology/molecular laboratories in large, tertiary medical centers. In this issue of the Journal, Xu and colleagues 1 have demonstrated the advantages of using a multiplex PCR assay to diagnose respiratory viral infections in a core laboratory serving a patient population presenting to the emergency department. Performance of the panel required a hands-on time of …

Development of Multiplex Reverse Transcription-Polymerase Chain Reaction for Simultaneous Detection of Influenza A, B and Adenoviruses
Background and objective:Millions of people in developing countries lose their lives due to acute respiratory infections, such as Influenza A & B and Adeno viruses. Given the importance of rapid identification of the virus, in this study the researchers attempted to design a method that enables detection of influenza A, B, and adenoviruses, quickly and simultaneously. ...
متن کاملDevelopment of a multiplex polymerase chain reaction assay for differentiation of field strain isolates and vaccine strains S19 and RB51 of Brucella in Iran
Bovine brucellosis is a zoonotic disease distributed worldwide and characterized by abortion and reducedfertility in cows. Since brucellosis eradication programme in Iran uses vaccination, test, slaughter andquarantine as control measures, it is essential to distinguish vaccine strains from strains that cause infectionsamong vaccinated cattle herds. We developed and evaluated a multiplex polyme...
متن کاملDetection Of Toxoplasma Gondii and Human Cytomegalovirus DNA in Blood from Transplant Recipients Using Multiplex Nested Polymerase Chain Reaction
Evidences from many studies suggested a polymerase chain reaction (PCR) as a valuable method for diagnosing infectious disease in the transplant recipients. We used this method for detection of Toxoplasma, gondii and human cytomegalovirus in blood specimens from patients after bone marrow or kidney transplantation. DNA of both infectious agents were detected using two separate sets of nested pr...

Background & Objective: Campylobacter can be transmitted through the raw milk. The purpose of this study was to determine the prevalence of Campylobacter genus and Campylobacter jejuni (C. jejuni) species in raw milk samples. Materials & Methods: In this study, 72 samples of raw milk were collected of the platforms milk in the Amol city in summer. Phenotypic identification of Campylobacter genu...
متن کاملGenotyping by multiplex polymerase chain reaction for detection of endemic hepatitis B virus transmission.
A nested polymerase chain reaction (PCR) protocol was developed for rapid genotyping of hepatitis B virus (HBV). During the first PCR round, a universal HBV primer pair was used to amplify the entire pre-S region of the HBV genome. Within the pre-S region, many nucleotide exchanges are observed. These are partly correlated to the serological hepatitis B surface antigen subtypes. Five additional...
متن کاملMultiplex Tetra-Primer Amplification Refractory Mutation System Polymerase Chain Reaction to Genotype SNP8NRG221533 of Neuregulin-1 Gene
Schizophrenia is a severe neuropsychiatric disorder with symptoms such as hallucination, delusion and mental disorder. It is a complex disorder, in which genetic components play a crucial role in its pathogenesis. Among candidate genes for schizophrenia, Neuregulin 1 (NRG1) gene is the most important gene, association of which with the illness has been confirmed in several studies. Single nucl...
